PHYSICAL THERAPY

Our physical therapists work with patients who have lost functional mobility due to accident, injuries, aging, amputation, birth conditions, and many other conditions.

At Century Homecare we believe that functional movement is a central part of being healthy. Our physical therapists work with you to address the illnesses or injuries limiting your abilities to move and perform functional activities in your daily life. Our physical therapists conduct a comprehensive initial assessment to establish a care plan in cooperation with your physician. The physical therapist will work with you towards those functional goals in a comprehensive but compassionate approach using tried and true treatment methods.

OCCUPATIONAL THERAPY

Our occupational therapy team helps patients participate in the things they want and need to do through the therapeutic use of everyday activities. This includes helping patients recovering from an injury to regain fine motor skills, and providing supports for older adults experiencing physical and cognitive changes.

After the initial assessment, our occupational therapist works with your physician and your caregivers to develop an individualized care plan that takes into account your goals and the final desired outcomes. The therapist then customizes a treatment plan to help you achieve the desired outcome. Our occupational therapist will also conduct a comprehensive evaluation of your home, make recommendations for adaptive equipment, train you and your caregivers on how to use it, and guide and educate for family members and caregivers.

SPEECH THERAPY

Our speech-language pathologists (SLP’s) work to prevent, assess, diagnose, and treat speech, language, social communication, cognitive-communication, and swallowing disorders result from a variety of causes, such as a stroke, brain injury, hearing loss, developmental delay, Parkinson’s disease, cerebral palsy, or emotional issues.

After the initial screening for communication and swallowing disorders, our speech therapist works with your physician to develop a plan of care suited to your needs and goals. The therapist periodically reviews the care plan against the goals and adjusts the treatment accordingly, in consultation with you and the other caregivers.
